Forms

Here are a selection of forms you may occasionally need.

Completed forms can be returned to the school office in person, or by email to admin@coleridgeprimary.net, unless specified otherwise. All of the below forms can be completed electronically using the Fill & Sign option in Adobe Acrobat Reader, or equivalent tool in your PDF reader of choice.


Absence

Daily attendance at school is vital for your child to achieve their full potential, and permission for absence from school will only be granted for medical, religious or exceptional circumstances (please read our Attendance Policy on our Policies page for more details). To request absence from school during term time, please complete the Request Pupil Leave of Absence form.


Admissions

For our main admission procedure, please see our Admissions pages. After you have accepted a place at Coleridge, we may send you our school Admission Form to gather necessary information about your child, and if you are starting in Reception, you may also received either a New Student Questionnaire or an Existing Student Questionnaire.


Change of Contact Details

It’s very important that we keep all of your information, including contact details and home address, up to date on our school systems, especially in the case of an emergency. Should you move house, change phone number or email address, please complete the Information Update form.


Lunches

We encourage all our students to partake in the healthy and balanced hot school lunch provided on site, but understand some parents prefer to make their own child’s lunch. If you are changing between school dinners and packed lunch, or vice versa, please let the school office know by completing the Change of Lunch form.


Photo Consent

We partake in a wide variety of activities on and off school grounds, which we sometimes like to share photos of with parents and friends of the school on our website, our social media channels, and in promotional material. Children are never identified by name in these images. You should have received a form for this directly, but if for some reason you have not or need to change your preference, please complete and return the Photo Consent form.


Trips Consent

The school takes children on a variety of school trips and extra activities throughout the school year, and we encourage everyone to attend. To take children out of school on trips, we do require the parents consent, which is given once at the start of each year, and covers all trips for the year. You should receive a form to sign directly, but it for some reason you have not, please complete and return the Trips Consent form.
